I attest that I am a healthcare provider (MD, DO, PA, RN, FNP, NP, or APN) with an active license and have sufficient knowledge, experience, expertise, qualifications, training, or medical specialty necessary to manage patients with rare diseases that cause obesity.

I consent to my name, professional and contact information being included in the Healthcare Provider Locator Tool for as long as Rhythm Pharmaceuticals shall use such information or until I expressly revoke my consent in writing to Rhythm Pharmaceuticals. I affirm that inclusion of the information I provide in the Healthcare Provider Locator Tool is consistent with any obligations, contractual or otherwise, that I may have. I agree to allow Rhythm Pharmaceuticals to publish and share my information for the purpose of making the information available to patients and caregivers.
